Insurance Coverage:

• We are in most HMO and managed care plans.
  
• We are contracted with many PPO plans.
• We also accept points of service (POS) plans and traditional indemnity plans.
• We attempt to contact all patients' insurance companies for verification of coverage prior to their first visit.
• We also bill workman's compensation and vehicular claims (auto accidents).
  

While most of our physicians are on many plans, please call our office to verify as an individual physician may not be in a specific plan. Further, while we will do our best to verify your coverage, you should always personally verify your benefits.

Once our physician determines your treatment plan, we will, upon your permission, share this information with your referring provider (i.e. primary care physician or specialist such as orthopedic surgeon). If you are covered by Worker's Compensation or vehicular claim (car accident), we will coordinate your coverage with your nurse case manager or adjuster/representative. If you have any questions, ask our scheduling staff when you call for your appointment.
